What is a root canal treatment?

Root canal treatment is the process of medically removing a seriously injured or diseased tooth. Also known as endodontic treatment, a root canal is the best way to relieve serious pain and make teeth healthy again. In addition to relieving pain, a root canal also helps protect surrounding teeth from excessive wear or strain.

Does a Root canal treatment hurt?

Since patients are given anesthesia, a root canal isn’t more painful than a regular dental procedure, such as a filling or getting a wisdom tooth removed. However, a root canal is generally a bit sore or numb after the procedure, and can even cause mild discomfort for a few days.

How do you know if you need a root canal treatment?

Root canal treatments are needed for a cracked tooth from injury or genetics, a deep cavity, or issues from a previous filling. Patients generally need a root canal treatment, when they notice their teeth are sensitive, particularly to hot and cold sensations. Sensitive teeth indicate that harmful bacteria are getting into the nerves of the jaw and must be removed.

Can I go to school or work after getting a root canal treatment?

Although you will most likely be numb for 2-4 hours following the procedure, most patients are able to return to school or work directly following a root canal treatment. However, most physicians advise against eating until the numbness is completely gone. If you work in manual labor, make sure to protect your teeth.

How long does a root canal procedure take?

Although it varies based on the type of tooth, a root canal takes roughly 90 minutes to complete per tooth. Most anterior teeth can be completed in one hour, while premolars and molars can take up to two hours to complete. For re-treatment, procedure times can vary.

Common Causes of Tooth Wear

GRINDING AND CLENCHING

If you grind or clench your teeth when you're feeling stressed or anxious, or at night in your sleep, this can cause the enamel to wear down prematurely. When you grind one tooth against another, the friction can slowly rub away the enamel. Clenching your jaw leads to the formation of micro-cracks in the tooth enamel. The enamel around these cracks then wears away more easily when you chew or brush your teeth.

Similar to how sandpaper wears away at wood, chewing on hard items like fingernails and pen caps can slowly take away the enamel on your teeth. Brushing too hard with an overly stiff toothbrush can have a similar effect. This is one reason dentists generally recommend using a soft toothbrush and a gentle touch when brushing.

ACIDIC FOODS AND BEVERAGES

When you consume acidic foods or beverages, the acid weakens your enamel. Then the enamel is more easily worn away when you brush your teeth or chew harder foods. Common acidic foods and beverages include sodas, citrus juices, and tomato sauce. Those with bulimia also experience acid wear due to frequent exposure of their teeth to acidic stomach contents.

Problems Caused by Worn Teeth

Many patients first notice their teeth are showing wear when they look in the mirror. However, excessive tooth wear can cause an array of other issues that you may not immediately realize are associated with the wear.

YELLOW TEETH

Your enamel is naturally quite white, but the layer beneath it—the dentin—is yellowish. As your enamel wears away and becomes thinner, the dentin starts showing through, and your teeth develop a more yellowed appearance. This is usually most obvious along the chewing surfaces of the teeth.

SENSITIVITY

DENTAL BONDING

Dental bonding is a procedure in which a tooth-colored composite material will be applied to the edge of your teeth. The composite will cover the worn surfaces, improving the appearance of your teeth and also addressing any bite alignment and sensitivity issues you may be experiencing. Dental bonding is usually the best option for minor to moderate enamel erosion in isolated spots.

CROWNS

If certain teeth are very badly worn on several surfaces, we may recommend covering those teeth with crowns. A crown is typically made from porcelain or composite resin. It covers the entire tooth, helping to stop any chips or cracks from spreading while protecting the tooth from additional enamel damage.

We may also recommend that you undergo fluoride treatments or use a fluoride rinse to harden your enamel and help prevent subsequent wear.

Gingivitis affects most people at some time during their life. It is usually caused by a build-up of plaque on teeth. There are two main stages of gum disease, Gingivitis and Periodontitis. 

Gingivitis is early gum disease and occurs when dental plaque builds up on teeth, particularly where the gum joins the tooth. Periodontitis is an advanced gum disease that may occur if gingivitis is not treated. The gum margin, the part of the gum that seals to the tooth, is weakened and spaces form between the tooth and the gum. These spaces are called ‘periodontal pockets.’ Bacteria become trapped in these pockets causing further inflammation (redness and swelling).

Wisdom Teeth 

Wisdom teeth are the molars (back teeth) that come through last, usually in your late teens or early 20s. There are normally four wisdom teeth – two in the upper jaw and two in the lower jaw – but some people have more, fewer, or even none at all. Sometimes, a wisdom tooth comes through at an angle and pushes into the gum or the tooth beside it. This is called impaction, and it can be painful and make the area difficult to keep clean.

Problems with wisdom teeth

When there is not enough room in the jaw for wisdom teeth, it can stop them from coming through properly. Brushing these teeth can be difficult. Food and bacteria can get stuck between the wisdom tooth and the tooth next to it, leading to tooth decay and gum infections. Crowded wisdom teeth in the upper jaw often lean sideways and rub against the cheek. This may cause ulcers on the cheek and chewing problems. 

Signs of gum infection caused by wisdom teeth:

  • swollen and sore lymph nodes underneath the jaw

Wisdom teeth removal

X-rays will help to tell if wisdom teeth might cause problems. Some wisdom teeth do not cause problems and do not need to be taken out. Sometimes, the dentist or oral surgeon may just need to cut a small part of the gum to help the wisdom tooth come through. 

If wisdom teeth do need to be taken out, most commonly a local anesthetic will be used, so you won’t feel it. Sometimes, if the wisdom teeth are deeply impacted (blocked from coming through), or for other medical reasons, a general anesthetic will be offered to the patient. Because wisdom teeth are large, the hole where the tooth was may be stitched to help it heal. It is normal for your jaw and gum to be sore, swollen, and bleed for a few days after removing a wisdom tooth. 

One possible complication of wisdom tooth removal is a dry socket (alveolar osteitis) when the area where the tooth came from doesn’t heal properly. Bad breath and severe pain from the socket are signs. A dry socket can be successfully treated by your oral health professional.
